How Go Clinic MD, LLC collects, uses, shares, and protects information from visitors of our public websites and mobile applications.
Go Clinic MD, LLC, on its own behalf and on behalf of its affiliates, makes certain telehealth-related information available to you and facilitates your access to telemedicine, concussion and traumatic brain injury (TBI) care, and related medical services (the “Go Clinic Services”). Medical care under the Go Clinic brand is provided by independent, physician-owned medical practices; Go Clinic MD, LLC does not itself provide medical care.
This Privacy Policy applies to visitors of the following publicly available websites: www.goclinicmd.com and the publicly available associated domains and mobile applications owned and operated by Go Clinic MD, LLC (collectively, the “Websites”). For purposes of this Privacy Policy, “you” and “your” means you as a visitor of the Websites.
This Privacy Policy describes the types of Personal Information we collect from visitors of our public Websites and our practices for using, maintaining, sharing, and protecting it. It also describes the rights and choices you may have with respect to your Personal Information and how you may contact us.
This Privacy Policy does not apply to information collected from visitors who register and log in (“Members”) to the password-protected and secure portions of our sites (“Secure Platforms”). All information collected and stored by Go Clinic MD, LLC or added by Members into the Secure Platforms is considered Protected Health Information (“PHI”) and/or medical information and is governed by the laws that apply to that information, including the Health Insurance Portability and Accountability Act (HIPAA), and by the Go Clinic Notice of Privacy Practices — not this Privacy Policy. Go Clinic MD, LLC will not use or disclose information received from the Go Clinic mobile app for advertising, marketing, or other use-based data-mining purposes, will not combine it with other data, and will not sell it. The Notice of Privacy Practices (and not this Privacy Policy) also applies to Personal Information collected on our Websites if specific symptoms or health conditions are also collected, as such information may be considered PHI under HIPAA.

The Websites may use cookies for various purposes, including to keep the information you enter on multiple pages together. Some of the cookies we use are “session” cookies, which are automatically deleted after you close your browser at the end of your session; these help optimize performance of the Websites. SMS opt-in phone numbers will NOT be shared with third parties or affiliates for marketing purposes. We may share phone numbers with service providers solely for the purpose of delivering SMS messages.

Both Go Clinic and the third party may have access to information about you and your use of the Websites and the third-party service. Go Clinic has no control over and is not responsible for the privacy practices of such third parties. We recommend that you familiarize yourself with the privacy practices of those third parties.
Our Websites may contain links to other online platforms operated by third parties. We do not control those platforms and are not responsible for their content, their privacy policies, or their use of your information. Information you provide on public or semi-public venues, including third-party social networking platforms, may be viewable by other users without limitation as to its use. Our inclusion of a link does not, by itself, imply any endorsement. Any information you submit directly to these third parties is subject to that third party’s privacy policy.
Cookies help our Websites function and let us understand site usage. You can restrict or delete cookies through your browser settings; the method varies by browser (Microsoft Edge, Mozilla Firefox, Google Chrome, Apple Safari, and others each provide controls). Residual copies may be retained in our backup and archival systems for a limited period, after which the information is automatically deleted or put beyond use where deletion is not possible.
The Websites are controlled and operated by us from the United States and are not intended to subject us to the laws or jurisdiction of any state, country, or territory other than that of the United States. Any information you provide through our Websites may be stored, processed, transferred between, and accessed from the United States and other countries that may not guarantee the same level of data protection as the one in which you reside. Go Clinic MD, LLC does not collect or share Personal Information on the Websites from any person we know to be under the age of 16. Go Clinic MD, LLC does not intentionally collect Sensitive Personal Information, as defined under the CCPA/CPRA and the VCDPA, from visitors to our public Websites, and does not use any such information to infer characteristics about a consumer.
Go Clinic MD, LLC collects Personal Information from the following categories of sources: directly and indirectly from you, including through your interaction with the Websites; social media platforms; third-party partners such as analytics providers; and automatically through tracking technologies. Go Clinic MD, LLC discloses Personal Information to the following categories of third parties for a business purpose, to comply with the law, in the context of a transaction, or for other business reasons: identifiers (IP address, device ID, online identifier, and, where you provided it, name, email, date of birth, phone number, and address) to service providers and data analytics providers; commercial information to service providers; and Internet or other electronic network activity information to service providers and data analytics providers. Go Clinic MD, LLC does not sell Personal Information and does not share Personal Information for cross-context behavioral (targeted) advertising.

To submit a request to exercise any of your rights, please complete and submit the webform at [insert your consumer-rights request URL]. As required under applicable law, we must take steps to verify your request before we can provide Personal Information to you, delete Personal Information, or otherwise process your request. To verify your request, we may require you to provide your name, physical address, email address, contact information, and information about your account or previous transactions with us. We will deliver Personal Information that we are required by law to disclose within 45 days after receipt of a verifiable request, unless we notify you that we require additional time.
