For referral partners
Refer patients with a concussion or mild TBI, symptoms that are lasting longer than expected, or a neck or back injury from the same accident. The care team takes the case from intake to return to work, school and activity.

The right specialist first
After intake, the case is routed to neurology or PM&R, whichever fits it best, for a telemedicine evaluation.
One coordinated plan
Neurology, PM&R and the rehab team work from one chart. In-person therapy, imaging and testing are coordinated near the patient when the case calls for it.
Clean handoffs
Evaluations, scores and treatment notes stay in one chart across the team, with clean handoffs back to you whenever needed.
